Quality Assurance Guidelines
The HCAHPS
Quality Assurance Guidelines Version 3.0 has been released.
This manual has been revised from Version 2.0 and includes additional updates and
enhancements that provide a comprehensive resource for hospitals and survey vendors
participating in the HCAHPS initiative. Please use
Version 3.0 going forward.

Quality Assurance Plan Specifications
Quality Assurance Plan Template and Specifications Document
Hospitals self-administering the HCAHPS Survey, hospitals administering the survey
for multiple sites, and approved Survey Vendors must develop a Quality Assurance
Plan (QAP).
The QAP should provide documentation of the hospital's or vendor's understanding,
application and compliance with the HCAHPS Quality Assurance Guidelines, and should
serve as the organization-specific guide for administering and training project
staff and subcontractors to conduct the HCAHPS survey. A comprehensive QAP, reviewed
and approved by HCAHPS, is required for participation in the HCAHPS program.
The HCAHPS Project Team has developed an outline of items to address when you prepare
and submit your QAP. The
Quality Assurance Plan Outline, is contained in Appendix
S of the
Quality Assurance Guidelines Version 3.0. We recommend that you make every
effort to address all of the items listed in the outline, in the order in which
they are presented.
Note: Any revisions to your QAP should be submitted using "tracked
changes."
